How to print questionnaires for patients.

Set Up

• Go into Lists
• Click on “Questionnaires” – near bottom of second column
• Insert a questionnaire name, question number and question.
• You can have as many questions as you want and lots of different questionnaires.

Printing a questionnaire

• Go to patient details for a patient
• Choose a questionnaire from the dropdown list (close to the bottom right)
• Click on P to print or V to view the questionnaire
