If you need to submit a GOS 3 or 4 for uncollected spectacles you need to do as follows:

• If using Opticabase Dispensing or Clinical, tick “Unc” just below the collection date box on the dispensing screen. Don’t enter a collection date.
• Go to the eGOS 3/4 form and click on “Fill Back”
• Do NOT sign for the patient. That should be left blank
• Click on “Uncollected” so it says “YES” in the box
• Sign as the supplier
• Click on “Submit Now”
